letter to insurance
I remember reading that someone (I think it was
Phyllis.) was offering to share a copy of the letter she
sent to her insurance company. I would like a copy, if
you’re still willing to share. My insurance (Cigna HMO)
is asking for more information than the doctor sent,
and I thought I would send along a personal letter as
well.
