I don't have all of the answers, I am still pre-op, but I have done a ton of
research.....here is a great site if you haven't been yet -
www.obesityhelp.com.

Here are a few answers I got from my doctor and insurance company:

1) Criteria - Most Insurance companies criteria is @ least a BMI of 40, 100
lbs over your ideal body weight (some require twice the ideal body weight)
plus other medical reasons (high blood pressure, heart disease etc.).

2) NO

3) Yes

4) Sugar (however some people say after a while, they can handle sugar in
small amounts), Nuts and Coconut

5) Total weight loss should be 1/3 of your pre-op weight.
