If you are so fortunate that your insurance company does not require
pre-certification for surgery, then you won't have to submit a letter. (Boy,
the rest of us will be so jealous!). Of course talk to your PCP. If your PCP
doesn't support your decision, now's the time to find out about it. You don't
need her permission, although it's good to have her support. If your insurance
requires a letter from her, though, you will need her permission - or you'll
need to find a new PCP.

I have tried all kinds of diets and one prescription weight lost
drug: from Atkins to diabetic diets (although I am not diabetic),
slim fast, xenical (which cost me over $100 and I could not take do
to the side effects).

I now feel extrememly out of control with my weight and feel that
this is the perfect time to begin asking my doctor about weight loss
surgery. If I don't do something now, I know that my weight will
continue to climb.

What I need to know is do I just bring up the topic of weight loss
surgery to my doctor or do I say that this is something that I have
thought long and hard about (which I have)? If my doctor agrees with
me, what happens next. I have read that I must submit a letter to my
insurance company (what the heck is that for, I don't remember
<!--more-->
submitting a letter to my insurance company for any surgery I have
ever had in my life).

Currently I am 100 pounds over weight and my BMI is exactly 40. I
don't have high blood pressure, but when I climb stairs I am finding
myself getting winded much faster than before.

I would appreciate someone giving me some advice regarding what I
need to do from informing my doctor (does he then refer me to a
surgeon), insurance company etc.

Thank you so much for your help.
