Living Wills and POA forms
Hi Everyone,
I was reading one of the postings which mentioned the importance of
havng a living will and a power of attorney for medical reasons. I
decided to look on the internet to see if I could find the forms at
no cost to me and I got lucky….found a site and printed off the
needed forms…so I thought I would share the site address with you
all….it is:
At this site you just go in to the site by a couple of screens and
there is a page that has a list of all the states…ou click on your
state and it brings up the document needed in your state. It is a
real easy site to navigate…

Thanks for looking into this, I am on my way with all the help from the others
at this site. I am scheduled for surgery on Feb 3rd, and will have my will in
place in 2 days. Have an appointment with a notary on Monday… Have a great
